Integrated Behavioral Health
Behavioral health care built into primary care and medical settings.
HRR embeds licensed behavioral health clinicians directly into primary care and medical teams, so patients get support for stress, mood, sleep, substance use, and chronic-condition management in the same visit, from the same team.
How the model works
Our behavioral health consultants work as members of the primary care team. When a provider flags a concern, the patient is introduced to a behavioral health clinician that day, often in the same room. Visits are brief, focused, and built around the patient’s goals.
Same-day warm handoffs from the medical provider
Brief, evidence-based interventions that fit the pace of primary care
Screening with validated measures and follow-up tracking
Shared care plans and documentation inside the clinic’s workflow
Consultation with prescribers on the behavioral side of medical care
Where we deliver it
Primary care clinics, federal and military health programs, community health settings, and school-based programs. HRR’s integrated care experience includes the Behavioral Health Optimization Program (BHOP), which embeds behavioral health into primary care.
What organizations gain
Earlier identification of behavioral health needs
Fewer lost referrals, because care starts during the visit
More provider time for medical concerns
Measurement-based tracking of patient progress
Quality oversight from credentialing through outcome monitoring
